低位显微外科与腹腔镜精索静脉结扎术对精索静脉曲张性不育的疗效对比(1)
【摘要】 目的：分析精索静脉曲张(VC)不育治疗中低位显微外科治疗与腹腔镜精索结扎术的应用效果。方法：选取2013年10月-2015年10月本院收治的VC不育患者66例。按照随机数字表法将其分为显微组(接受低位显微外科治疗)和腹腔镜组(接受腹腔镜精索静脉结扎术治疗)，各33例。比较两组术前及术后1、6、12个月時的精液指标，包括精子密度、精子总数及(a+b)级精子比率；统计随访1年期间配偶妊娠情况及不良反应发生情况。结果：术前及术后1、6、12个月，两组精子密度、精子总数、(a+b)级精子比率比较，差异均无统计学意义(P>0.05)；两组术后精子密度、精子总数及(a+b)级精子比率均较术前呈显著的升高趋势，比较差异均有统计学意义(P<0.05)；显微组配偶妊娠18例(54.54%)，低于腹腔镜组的19例(57.57%)，但差异无统计学意义(P>0.05)；显微组不良反应发生率为3.03%，低于腹腔镜组的21.21%，比较差异有统计学意义( 字2=5.121，P=0.023)。结论：低位显微外科治疗与腹腔镜精索结扎术在VC不育的治疗中均有着较为理想的治疗效果，且低位显微外科治疗可有效降低不良反应的发生率。

【Abstract】 Objective：To analyze the effect of low microsurgical treatment and laparoscopic varicocele ligation on varicocele(VC) infertility.Method：A total of 66 infertile patients with VC in our hospital from October 2013 to October 2015 were selected.According to the random number table method，they were divided into microscopical group(low microsurgical treatment) and laparoscopic group (laparoscopic varicocele ligation)，33 cases in each group.The semen indexes of two groups before and after operation 1，6 and 12 months were compared，included sperm density，total sperm count and(a+b) grade sperm ratio，the status of spousal pregnancy and incidence of adverse reactions between two groups were statistically followed up during the 1 year period.Result：The sperm density，total sperm count and(a+b) grade sperm ratio in two groups before and after operation 1，6 and 12 months were compared，the differences were not statistically significant(P>0.05).After operation，the sperm density，total sperm count and(a+b) grade sperm ratio in two groups were significantly higher than those of before operation，the differences were statistically significant(P<0.05).The spousal pregnancy in microscopical group was 18 cases(54.54%)，which was lower than 19cases(57.57%) in laparoscopy group，but the difference was not statistically significant(P>0.05).The total incidence of adverse reactions in microscopical group was 3.03%，which was lower than 21.21% in laparoscope group，the difference was statistically significant( 字2=5.121，P=0.023).Conclusion：Low microsurgical treatment and laparoscopic varicocele ligation have an ideal therapeutic effect in treatment of VC infertility，and low microsurgical treatment can effectively reduce the incidence of adverse reactions.
